Lash Extension Terms for ALL CLIENTS

From the entire Lashfab Team, thank you for being a wonderful client.

We try our best to keep our policies very simple, we focus on the happiness of our members first and foremost which is why we only have twelve central policy items. Other terms and conditions are available on our website at http://www.hellolashfab.com/terms

Terms: I understand the following rules and contractual obligations apply to any lash extension services done at HELLO LASHFAB LLC

1. My Lash Extension Fill Package or Membership will consist of ongoing monthly payments of $129, $149, or $179 depending on the service level.

2. Each membership or package of services consists of 2 monthly fills that must be used during that month, unless an exception is further outlined in this agreement. Any additional fills will be at an additional cost.

3. With 90 days notice, this agreement can be canceled with written notice via email to info@hellolashfab.com. YOU WILL BE RESPONSIBLE FOR YOUR MEMBERSHIP PAYMENTS UNTIL THE CANCELLATION PERIOD IS OVER and any unused fills will be automatically voided on the cancellation date.

4. Contracts cannot be transferred to any other person or entity. This contract is for you and you alone, these terms extend to any ongoing (2 or more consecutive services) client that visits our studio.

5. No refunds are given on paid monthly fees or unused fills.

6. You may cancel or reschedule an appointment with no charge if done 24 hours prior to the appointment time. If canceled or rescheduled within 24 hours of the appointment time, a $30 late cancelation fee will apply.

7. I understand my credit card, or any card on file with us (in the event the primary card is declined) will be charged for the monthly membership on the date of execution of this agreement.

8. An unused fill MAY roll over to the next month, up to 1 fill, once per year. This can be used as a good option for vacations or other events in which you are unable to be at the studio for your scheduled fill. WE MUST BE NOTIFIED IN ADVANCE AND IN WRITING.

9. We recommend you make your appointments at least 4 weeks in advance to guarantee that you are seen on the exact time and day you’d like.

10. Members or clients receiving ongoing fills and services are required, per this Membership Agreement, to notify Hello Lashfab LLC in writing of any change in credit card or account information on file prior to the next payment date. Payment Authorization Forms are available at the front desk from an associate. Any non-payment will be sent to an outside collection agency after a 10 day grace period.

11. After a 10 day grace period, non-payment of membership or ongoing service fees will result in an automatic cancellation of future appointments and the full cancellation period (3 months) will be assessed and billed. The total balance owed will be sent to collections within 30 days of delinquency. This will be reported to the credit agencies.

12. To change my monthly payment date, a prorated amount for the time difference will be applied to my account.
